Very interesting study!
Attached you will find the questionnaires we use in the adolescent psychotherapy study FEST-IT. See link below. Unfortunately they are in Norwegian, however, not very difficult to translate to Swedish, if that's an option. In FEST-IT we use the first two at post-treatment and all three at 1 year follow up.
